When to Use
Under a hybrid working model, employees whose role is suitable for this way of working split their time between working remotely and physically attending the workplace.
This form should be used when an employee has been granted the opportunity to work under a hybrid work arrangement. This form agreement should be used in conjunction with a Hybrid Work Policy and, if applicable, a Hybrid Work Arrangement Request Form.
