When to Use

The Pregnant Workers Fairness Act (PWFA) requires an employer with 15 or more employees to reasonably accommodate qualified employees and applicants with known limitations related to pregnancy, childbirth, or related medical conditions, barring undue hardship. 

An employer may use this form to request documentation supporting an employee's accommodation request under the PWFA only when it is reasonable under the circumstances to determine whether the employee has a limitation and needs a change at work due to the limitation. An employer should thoroughly understand when it is reasonable and not reasonable under the PWFA to request supporting documentation before using this or any form to make such a request.
