When to Use
The Americans with Disabilities Act (ADA) requires an employer to provide an employee with a disability a reasonable accommodation to help the employee perform the essential job functions. An accommodation is not reasonable if it causes a significant difficulty or expense (i.e., undue hardship) on the employer's business. This form will help an employer obtain information from the employee concerning their accommodation request, including relevant limitations, the impact of the limitations on the performance of essential job functions, the nature of the requested accommodation and employee health care provider information.
