When to Use

Employers should utilize this form to notify employees who have requested a religious accommodation of the outcome of the employee's request. This form provides the employer with an opportunity to detail the accommodation being provided or the reasons for denying the requested accommodation. The form also allows the employee to respond to the employer's decision, thereby allowing further dialogue where warranted. Employers should provide a copy of this form to the employee and keep a copy for their records.
