Designation of FMLA Leave Letter

When to Use
Employers are responsible for either designating an employee's leave as qualifying for Family and Medical Leave Act (FMLA) leave or refusing to designate it as such. Such designation must be in writing. Employers should use this letter when they have received a fully completed certification from an employee's health care provider and determined that the employee's request for leave qualifies as FMLA leave.
