When to Use
An employer should use this letter when an employee fails to timely return previously requested certification supporting the need for leave under the Family and Medical Leave Act (FMLA) for the employee's or a family member's serious health condition, to care for a seriously ill or injured military servicemember or for a qualifying exigency.
