When to Use
An employer should use this letter when an employee's certification for leave under the Family and Medical Leave Act (FMLA), for nonmilitary-related reasons, is incomplete (i.e., one or more entries have not been completed) or insufficient (i.e., all entries have been completed, but the information provided is vague, ambiguous or nonresponsive).
