An employer may use this form to signify that the employee has received and reviewed a copy of the company drug and alcohol testing policy, and acknowledges and consents to its terms. The Acknowledgement and Consent Form for Drug and Alcohol Testing should be used in conjunction with the Drug and Alcohol Testing Policy.
